Speaker Information Form

This form is for confirmed panel speakers and chairs at College events.

The information you provide will help us plan and coordinate logistics for the event. Some information may also be used for promotional purposes, if indicated.

Dinner: All speakers are invited to join the College Guest Night Dinner after the event (approx. 7.15-9pm). This is a “smart attire” sit-down dinner event with students and other members of Kellogg. Will you attend the College Guest Night Dinner after the event?
Travel: To help us plan our budget, could you please let us know whether you expect to claim travel expenses for this event? (You will be given an expenses form and guidance, if so.)
Accommodation: Speakers coming from afar may reserve a room in College accommodation, subject to availability. Would you like to reserve a College room?
In principle, do you give permission for this event to be filmed or photographed? (Permission will be sought on the day; you can always change your mind.)
